| Do I have to re-register if I have services at multiple locations? |
Throughout the remainder of 2008, we will be transitioning to Electronic Healthcare Records (EHR) for our patients. This system will allow your information to be available at all of our clinic locations, further improving efficiency and quality of the healthcare you receive. Until this transition is complete you may have to register at each Doctors Clinic.

| Will you bill my insurance? |
Yes. As a service to our patients, primary insurance is billed. We have recently enhanced our system to accommodate automatic billing of secondary insurance. Please be sure to keep us informed of your insurance coverage.

| What do I do if I receive two statements? |
As we upgrade our electronic systems it is possible for you to receive two statements of account. This will ONLY occur if you have a balance on your account as of July 15, 2008. Each statement reflects different dates of service. Services after July 15, 2008 will be reflected on a new and improved statement. We think you’ll appreciate the new format which is easier to read, provides detailed information and more closely matches up with your explanation of benefits (EOB) from your insurance carrier. Until your old account is paid off, please make payment on both statements. Remember to include stubs from each to ensure proper credit.

| What if I can't make my appointment and I need to reschedule? |
Please call your physician’s office as soon as you realize you will not be able to keep your appointment. We prefer all cancellations be made at least 24 hours prior to your scheduled appointment time. This will allow us to give another patient the opportunity to be seen.

| If I call with a question for the nurse or doctor, when can I expect a call back? |
Our physicians and nurses are with patients throughout the day, so it is usually not possible for them to immediately answer phone calls. We will return your call as soon as possible. To help us get back to you quickly leave us your full name, date of birth, reason for the call and current phone number.

| Do you have after hours care? |
If you are having a life threatening emergency, call 911.
If you call after clinic hours, the answering service will receive your call. They will take your name, phone number and nature of your call. The answering service will page the on-call physician who will call you back within 15 minutes.
When you receive the call back, the on-call physician will instruct you as to whether you need to go to an emergency room, urgent care clinic or if the problem can wait for an appointment the next day. Have your prescription medication information available during this call.
In the rare instance of not receiving a call back within 15 minutes of your original call, you are encouraged to call the answering service again.
