How to Request a Physician Connect Account:

• Click here to review the PC system requirements
• Print, read the Access Agreement and return a signed copy of the first page for your medical practice or entity.
• Print and return the completed Physician Connect Access Request Document
• For each person requesting access return a signed copy of Appendix C
