When making appointments, our scheduling staff will need the following information:

The full legal name of the patient and, if applicable, the responsible party
The name of the physician or provider you wish to see
The nature of your health problem or why you wish to see the physician or provider
Your primary and secondary insurance coverage
Your phone number for confirming your appointment

If you are a new patient to the Clinic, we will ask you to complete a registration packet which will include:

a) Patient Information Sheet
b) Medical History Form
c) Any other forms pertinent to your care or insurance plan

If you wish to have these forms mailed to your home prior to your visit (time allowing), please ask the Appointment Scheduler to mail you a packet. Please bring the completed forms with you to your appointment. If you have not received these forms or do not have these forms completed, please arrive 15 minutes early for your appointment to complete the registration process.

We appreciate receiving 24 hours' notice if you are unable to keep your appointment. This will allow us to give another patient the opportunity to be seen.

What You Should Bring to an Appointment

Please bring:

Your insurance card and/or coupon
Your copay (if you have one) Note: Copays are required to be paid at the time of service
Change of address or phone number(s) if you’ve recently moved or made a change
If you do not have insurance, you will be required to pay a $98.00 deposit towards your services prior to your appointment

COVERAGE REMINDER: If you are scheduling a regular well exam and additional care is provided other than what is normally covered in routine well exams, your insurance may require an additional copay or deny payment and you will be billed for these services. Please check with your insurance company for more information about your coverage.
